Please visit a store affiliated with HOT HOSYOU INC. near you fill in the required documents and apply with necessary documents.

Documents required if the applicant is an individual
・Identity verification documents (One of the following)
→ Driver's license / Pension certificate / My number card / Residence card / Passport / Health insurance card
(Additional documents are required in the following cases)
・ Minor → Parental consent form / Certificate of registered seal
・ Welfare recipient → Documents that can confirm receipt
Documents required if the applicant is a corporation
・Certified copy of commercial registration
※Other documents may be required during screening.

[Screening]

Screening takes about 30 minutes to 1 hour and the results will be informed to the store affiliated with us. In addition, we may contact the contact information provided at the time of application to confirm the application details and intentions.

Please prepare the following applicable documents and sign the contract.

Documents required if the applicant is an individual
・Certificate of registered seal of joint guarantor
※The contract must be stamped with the joint guarantor's registered seal.
Documents required if the applicant is a corporation
・Company’s certificate of registered seal
・Certificate of registered seal of joint guarantor
・Information provision completion confirmation document
※The contract must be stamped with both the company’s and the joint guarantor's registered seal.
